Is the Speaker icon left end of the sound bar crossed through. If it is Tap to turn spealker on.
Also could you just try Pressing the Phones volume + button when you expect an Instruction. Been known to resolve the problem even if Volume was okay on the phone prior to starting the App..
Is your phone connected to the infotainment system via Bluetooth, or is BT on? If so, all navigation instructions will go to the BT channel and your phone will be silent. This is a known problem that TT refuse to correct. If you need BT and it's linked to the infotainment, go into the phone settings, find BT, find the entry for your car infotainment, in the settings untick media audio (your phone settings may differ, but you get the idea).

If my iPhone is connected to the Ford Sync in my Fiesta....
Then there are NO spoken instructions from the TT Go App....
Disconnect Ford Sync and the spoken instructions work on the TT GO App....
OP has already said he is not connecting to a car's infotainment system - I covered this is my response, because the Bluetooth issue is well-known ie if BT is on, no device sound
The problem sounds like a very tricky one - the standard advice will be to uninstall and then reinstall, which is a real pain, but it seems somehow it's the only cure
Since you're using version 2.3, Do you see the below option under the Main Menu screen (left bottom corner)? Clicking on it should populate the prompt to select the audio output source. Can you see 'car' as an output source?
I only ever have phone volume on low (catch-up radio before sleeping) so even 100% of Music volume is very quiet. I always forget to set Music volume high before a trip so always need to pull-in when on the road to increase Music volume. Any way of separating the TT instructions-volume from Music volume? iPhone SE with GO v2.3.

In iTunes on your phone is the music volume on as the Volume setting in Voices in TT Go App says the Voice volume as a % of the music volume.

What app version are you using on your iPhone?
